"I use Google Forms.
Use the embed code and put it in a wiki page.
Then all you have to do is share the link.
Not sure how to not allow repetitive voting. Google Docs may not be the answer for that.

.doc is Word 1997-2003
.docx is WOrd 2007
.rtf is Rich Text Format
