My school is looking to implement a summer math portfolio to help students maintain skills over the summer break. We're hoping to find an online program that would adjust to each student's performance level. Please share if you have used a program such as this or know of one.

Have you checked out SkillsTutor? I used it at the middle school level last year. There was a range of activities and lessons accessible online. I used it mainly for reteaching and extension opportunities but I don't see why it couldn't be used an alternative to a summer math packet. It doesn't adjust to students ability levels, although teachers/ moderators can decide which lessons the students must complete. Additionally, students can complete each lesson multiple times to ensure mastery.I am not sure of the cost per student or by district but it could be something to look in to. See link below.
