Looking for an Internet Policy Handout to Distribute to 1st Grade Parents

Hello friends! I am looking to add pictures and student work to my classroom website and I need to create an Internet Policy Handout that parents/guardians will need to sign. Does anyone know of a website or template that I can access in regarding this? If not, what would I need to state in the policy handout if I have to create one?

If you scroll down the bottom of the page, there is a 'click here' link under student privacy. Click it and you will see my release form for parents. Feel free to use what you want from that. I don't want to get into posting photos, so left that out, but I did discuss the option of posting student work.
