Study Skills and Learning to Learn

WANTED accessible (!) surveys for ESL students to fill out online re their study skills and learning to learn modules they are taking as part of their learning English classes. I'm hoping this exercise will give the students a real opportunity to reflect on their learning so far - but as yet, I haven't found anything pitched for adults that isn't too complicated. Any ideas?
