chronic care community corps

Chronic Care Community Corps seeks to build the capacity of our communities to care for elders with chronic illness. We believe that those who have cared for elder loved ones have wisdom to offer to those just beginning to provide care, and that if we share that wisdom, mobilize existing resources, and strengthen our communities, the caregiving experience will be both more informed and more coherent. Chronic Care Community Corps will equip community groups with strategies, knowledge, and assistance to help caregivers and care recipients more effectively navigate the journey of chronic illness both inside and outside of healthcare institutions.
