Internet consent forms

I was wondering if anyone out there has samples of what their school or Board uses as far as internet usage consent. We have one in our board, however, I'm not sure if it is the best out there. If you are willing to share I would appreciate it. thanks

This was my second year doing a blog and I started a wiki toward the end of the year. I have a SMART Board in my classroom so I try to integrate it most of the time. I would love to hear about some of the technology you are using. You are SO lucky that you got to go to NECC!!! Did your board pay for you to go? Was there any special provincial funding that I should know about? I followed it a bit from home. I'm already planning to go to ECOO in November and was hoping to go to the OTF Technology workshop this summer, but the timing didn't work out.
