Special Education teachers - do you have any lesson planning templates to share?

I am looking for examples of lesson planning templates that include the goals from the IEP, instructional materials/strategies and include time/date/place (inclusion) details. I believe this template would have involved some creative thinking from outside of the box. Has anyone taken the time to create something like this? Special education teachers have plans that are uniquely different from what a regular education teacher creates because of so much more individualization.

 Thank you Bob for posting these helpful websites.  I will be graduating in the Fall with a special edcuation degree and need all the help I can get with creating and modifying lesson plans. I am pretty nervous about my future first year of teaching will definently utilize this website.
