On-Line Portfolio samples

Can any of you point to web sites that serve as portfolios to demonstrate student competency and achievement in social skills, problem solving skills, and skills needed in the workplace? Do any of these sites collect information as a youth moves from 7th grade to 12th grade, or from one school to another?
